How to Create an Account

  • Enter appropriate information in the First Name, Last Name, Middle Name, fields.
  • Enter a Birthdate by using the Month, Day and Year drop-down lists.
  • (Optional) Select a Gender from the drop-down list.
  • Enter appropriate information in the Address, City, Country, State, and Zip Code fields.
  • Enter a Phone Number. Must be entered in a 10-digit format with no dashes, slashes, periods, etc. (Optional) Enter Health Notes.
  • (Optional). Enter an Emergency Contact Name.
  • (Optional) Enter an Emergency Contact Phone Number.
  • Enter an Email Address - This will be used as the account "Username" when logging in.
    • Enter a Password.
      A variety of Password rules apply.
    • Passwords must be 8-to-16 characters in length.
    • Passwords are case sensitive.
    • Passwords must contain at least one alphabetic character.
    • Passwords must contain at least one numeric character.
    • Passwords must contain at least one special character (!@#$%^&*-=).
    • Passwords must not contain any spaces.
  • Verify the Password.
  • Check the Opt-In checkbox to receive marketing campaigns in email.
  • Click the Add Member link next to the Family Members box.
    • A shortened version of the Create Account screen will appear:
      The Last Name field will automatically be filled in (to match the Primary Individual's Last Name)
      this information can be edited if desired.
    • Enter other additional information as necessary.
    • Hit the Continue button.
    • (Optional) Continue clicking the Add Member link as many times as necessary to add additional Family Members.
    Click the Submit button when finished.
